  • Emergency contact numbers if parents can not be reached. This may be a neighbor or friends or relatives the baby sitter can contact

  • The location, name and phone number of where the parents will be, whether it is work or restaurant etc

Write down the numbers of:

  • Poison Control
  • Police Department
  • Fire Department
  • Hospital
  • Doctor’s name and phone number

Information about Children

  • Children’s names and ages
  • Foods that are allowed or not allowed
  • Television programs and movies that are acceptable and unacceptable and how long they can watch
  • Bedtime times and routine – remember to include if your child likes a special toy, night light, door open etc
  • Special needs or medications, including allergies. Include instructions for the location and administering of medicines
  • Areas in the house or types of play that are out of bounds
  • List some activities and books your children enjoy
